Navigating Chronic Disease Management and Dignity of Risk: Strategies for Aging and Disability Services

This training will cover two intersecting topics: Chronic Diseases and Dignity of Risk. It will equip case managers working in aging and disability services and adult protective services with the knowledge and skills to support clients managing chronic diseases. Participants will explore the impact of common chronic conditions on physical, emotional, and social well-being, while learning strategies to promote self-management, prevent complications, and enhance quality of life. In addition, participants will leave the training with a deeper understanding of how to have conversations around suicide and chronic illness empathetically and ethically, equipped with practical skills to provide meaningful support in their clients.

Learning objectives:

By the end of the training, staff will be able to

  1. Identify the most common chronic conditions affecting aging and disabled populations and their risk factors

  2. Recognize the physical, psychological and social impacts of chronic diseases on clients’ daily lives

  3. Understand how to connect clients with community resources for addressing chronic diseases

  4. Learn realistic tools for managing chronic conditions

  5. Articulate the concept of "Dignity of Risk," explaining how it applies to clients' autonomy in managing their chronic diseases

  6. Identify and differentiate passive suicidal thoughts from active suicidal ideation and assess clients' mental health status and the best way to move forward

  7. Practice communication techniques that foster trust and openness

  8. Understand the ethical implications of supporting clients who express thoughts of self-harm or assisted suicide and the importance of balancing client autonomy with safety concerns

Presented by: Hannah Linden, MPH, CHES, and Shelby Greene LCSW

Hannah Linden, MPH, CHES.

Hannah serves as the Wellness Director at the UNR Sanford Center for Aging, where she works on a statewide level to address gaps in chronic disease prevention and management, while improving access to evidence-based health education for older adults in Nevada. She oversees wellness programs, including chronic disease self-management, falls prevention and caregiver programs. She also works closely with participants in various programs to provide them with the tools and resources needed to improve their health and well-being. Before this role, Hannah coordinated a program at Community Health Alliance to improve family health and lifestyle habits and taught undergraduate courses in the UNR School of Public Health.

Shelby Greene LCSW.

Shelby is a Licensed Clinical Social Worker. Shelby works as the Coordinator of Clinical Services at the University of Nevada, Reno School of Medicine's Sanford Center for Aging. In this role, Shelby currently provides field instruction supervision for BSW and MSW students, in the pass Shelby has lead program development and management for Chronic Care Management while conducting psychosocial assessments as part of an interdisciplinary clinical geriatric assessment team. Shelby also takes charge of interprofessional education initiatives, fostering a collaborative environment for student growth that fosters a holistic patient care approach. Over the last several years, Shelby has worked directly with School of Social Work and has demonstrated a commitment to education and mentorship; from overseeing BSW and MSW students' field experiences as an off-site supervisor, to instructing content core courses in the Master Social Work Program. Shelby is licensed and in accordance with regulations outlined by the State of Nevada Board of Examiners for Social Workers, Shelby strives to empower the next generation of social workers and provide learning opportunities within the healthcare field. Shelby has worked with vulnerable populations and is very knowledgeable when navigating complex healthcare systems which is evident in various previous roles, whether facilitating co-occurring IOP group therapy sessions at Reno Behavioral Healthcare Hospital or advocating for patients' post-discharge needs at Renown Medical Center. Shelby is involved in professional organizations including Society of Social Work leadership in healthcare and community outreach initiatives.
