Description

This training will explore and assist in the development of a Quality Improvement Plan that will measure quality of service delivery in real time.  Specific components of a Plan will be provided along with tools to manage the plan overtime with the goal of improving services.  This training will also provide guidance on developing a Needs Assessment Plan utilizing internal data as well as external data.  Utilizing Needs Assessment data is critical to ensure you are meeting the needs of the community where services are provided.  Additionally, Needs Assessment demonstrates to key stakeholders and funders that services are matched to the community where services are provided.

Learning Objectives

At the conclusion of the workshop participants will have a working knowledge of: 

·         Key components of a Continuous Quality Improvement Plan

·         How to utilize tracking tools to ensure the Plan is being implemented effectively

·         How to create CQI Teams to review and identify areas that need improvement

·         Document findings and share with staff, Board of  Directors and Community Stakeholders

·         Key components of a Needs Assessment Plan

·         Steps to gather internal data utilizing the organization Electronic Health Record

·         Steps to gather external data that is readily available within the community services are provided in

·         How to document review of Needs Assessment data and to utilize data to improve client services
