Suicide risk rarely presents cleanly. This three-hour training provides clinicians with a practical framework for assessing and responding to suicide risk when substance use, psychosis, or both are present. Participants will learn to move beyond counting risk factors and develop a structured clinical formulation that integrates current suicidal thinking and behavior, acute changes, substance use, psychotic symptoms, protective factors, and foreseeable changes in risk. The training will also examine the different roles substance use may play in a clinical presentation and how to assess possible substance-induced versus primary psychotic symptoms when there may not be complete diagnostic certainty. A case example will be presented and revisited as new information emerges, allowing participants to practice assessment, formulation, intervention, and disposition decisions. The training will also address Nevada legal requirements, scope-of-practice boundaries, consultation, and documentation.

Learning Objectives

By the end of this presentation, participants will be able to:

  1. Conduct a focused suicide risk assessment that includes suicidal thoughts and behaviors, warning signs, acute changes, access to lethal means, protective factors, and relevant clinical context.

  2. Develop a risk formulation that integrates enduring vulnerabilities, recent precipitants, current suicidal state, current clinical state, protective factors and supports, and foreseeable changes in risk.

  3. Evaluate the role substance use may be playing in a clinical presentation and identify implications for assessment and intervention.

  4. Identify psychotic symptoms and substance-related changes that may increase or complicate suicide risk, and recognize when diagnostic uncertainty or clinical severity warrants consultation, referral, or a higher level of care.

  5. Develop an appropriate clinical response that may include collaborative safety planning, lethal-means counseling, involvement of supports, consultation, referral, and documentation consistent with scope of practice and relevant Nevada requirements.
