Description

This training will focus on screening, diagnosis, assessment of substance use and co-occurring treatment.  This training will address the latest evidence-based practices to integrated care. Coordination of care, case management, recovery support and other related activities will also be reviewed.  Lastly, continuity of care across the continuum will be reviewed as an important element in effective long lasting recovery.  

Learning Objectives

By the end of this presentation, participants will:

• Identify appropriate screening, diagnosis, and assessment practices for substance use and co-occurring disorders.

• Describe evidence-based practices that support integrated care.

• Explain the role of coordination of care, case management, and recovery support in treatment.

• Describe the importance of continuity of care across the continuum in supporting long-lasting recovery.
